SUPPORT COORDINATION
Your support coordinator is your link between the NDIA (National Disability Insurance Agency), the NDIS (National Disability Insurance Scheme), your plan manager (who controls your fund budget; this might be the NDIS itself, a registered, independent third party provider, or you yourself!) and your service providers (your therapy team, your care team, equipment suppliers etc.).
The role includes 4 main elements:
- Care management – your carers are with you almost every day; it’s important that they contribute to your life and your wellbeing productively & professionally. Our support coordinators will check in with you and with them regularly to ensure everything is going the way you want it to. We work carefully to plan & shedule your carer team to utilize your budget effectively & efficiently through the life of your plan, so you get the most benefit from these services.
- Therapy management – your support coordinator will liaise with & arrange your therapy teams schedule, and for the management of all associated invoices and document control. They’ll also ensure the therapists are following your therapy and budget plans, and always working towards your goals. We’ll get feedback from them regularly about progress towards your goals, we’ll manage any service delivery issues, & ensure the timely & thorough delivery of the critical end-of-plan reporting. We also handle administrative interactions with your plan manager & the NDIS, where required.
- Plan reviews & planning meetings – When the NDIS reviews your plan you get a chance to speak with them about the things that are working, and the things that could be working better. It’s a crucial time to be well prepared so you get the most benefit from your plan. Our coordinators will make sure the preparation for the meeting, and the meeting itself, are managed thoroughly & professionally. NDIS planning meetings happen at the beginning & end of each plan. This is where you & we meet your planner & discuss your disability needs, & they decide your budget. Our role (with your help, & that of your therapists & clinicians) is to provide the evidence the NDIS requires to make their decisions, then substantiate & clarify to the planner what you would like that to look like for your next plan budget.
